The Neurology of the Elderly

Traditional methods in the clinical practice of neurology have dominated clinical teaching in this specialty for about 100 years. Essentially these methods involve meticulous attention to detail and the recording of clinical facts. Thus the clinical history must be recorded chronologically, preferably in the patient's own words, and of the nervous sys­ followed by a carefully structured examination tem set out in such a way as to allow the precise localisation of the lesion or system involved. Clinical neurology taught and practised in this way has bred generations of neurologists throughout the world and raised the standards in the specialty to a level where clinical skills are probably unexcelled in any other specialty. With increasing availability and reliance upon non-invasive imaging techniques, the need for these skills in large areas of neurological practice has diminished. But perhaps more importantly, the classical clinical methods in neurology were developed when the elderly population was much smaller and when the specialty of geriatrics did not exist. As a result, much of the methodology is irrelevant or unreliable in of geriatrics will frequently the elderly population and the student find himself searching in vain in the textbooks of neurology for help in assessing an elderly patient with an atypical presentation (for example disturbance of balance or recurrent confusional episodes) of some common neurological disorder.

Neuropsychological and Behavioral Aspects of Diabetes (Contributions to Psychology and Medicine)

Behavioral medicine has blossomed as an area of systematic investiga­ tion during the past 10-20 years. Throughout its steady growth, there have been increasing interest and specialization in the study of neuro­ psychological and behavioral aspects of diabetes. This book attempts to capture and report exciting new developments in the study of both insulin-dependent (Type I) and non-insulin-dependent (Type II) dia­ betes mellitus. Accordingly, it is divided into two major sections. Physiological aspects of each disease, which differ significantly in pathophysiology and course, are discussed in separate medical over­ views that introduce each major section. These overviews are written by Drs. Tsalikian and Zimmerman, leading medical researchers in insulin­ and non-insulin-dependent diabetes, respectively. Each section also contains chapters describing neuropsychological and cognitive disease correlates, psychosocial patterns of adjustment, and treatment adher­ ence issues. Psychological aspects of insulin-dependent diabetes have been studied more extensively than non-insulin-dependent diabetes, perhaps because it is more often associated with graver medical compli­ cations. Therefore, there is a larger body of research to review and the first section has been divided into chapters on cognitive disease se­ quelae in populations of children and adults, separately. In his chapter, Dr. Ryan discusses developmental factors related to the unique sensi­ tivity of the brain to metabolic derangement. This book is written for the clinician, students, and practitioners of neuropsychology, neuropsychiatry, and behavioral neurology. It has been my intent throughout to present a synthesis of ideas and research findings. I have reviewed thousands of articles and research reports and have drawn extensively from diverse sources in philosophy, psychol­ ogy, neurology, neurosurgery, neuropsychiatry, physiology, and neuroanatomy in order to produce this text. Of course I have also drawn from my own experience as a clinician and research scientist in preparing this work and in this regard some of my own biases and interests are represented. I have long sought to understand the human mind and the phenomena we experience as conscious awareness. After many years of studying a variety of Western and Eastern psychologists and philosophers, including the Buddhist, Taoist, and Hindu philosophical systems, I began, while still an undergraduate student, to formulate my own theory of the mind. I felt, though, that what I had come upon were only pieces of half the puzzle. What I knew of the brain was minimal. Indeed, it came as quite a surprise when one day I came across the journal Brain as I was browsing through the periodicals section of the library. I was awed. An entire journal devoted to the brain was quite a revelation. Nevertheless, although intrigued by the possibilities, I resisted.

The publication of this anthology culminates what began as a Visiting Distinguished Scholars Lecture Series sponsored by the School of Jus­ tice Studies. When Dr. John M. Johnson was awarded the Arizona State University Graduate College's Distinguished Research Award for 1986- 1987, the School faculty voted to use the accompanying stipend to bring several scholars to campus. Each visiting scholar was commis­ sioned to present an original paper on contemporary issues in justice and to meet with graduate students and faculty during a week-long visit to campus. This collection of essays promotes wide-ranging conceptions of justice. As first conceived, we sought to bring an interdisciplinary per­ spective to the study of justice as a way of intellectually extending the current focus of research and teaching. As it developed, the collection permitted us to reflect on our own instructional program in law and the social sciences and to promote a conception of social conflict and control which includes social, political, economic, and legal controls.

The management of Alzheimer's Disease and the related dementias is one of the major challenges to health care professionals and American society-at-large for the coming decade and the coming millennium. The rapid growth of the over-eighty-five population, the group which, as recent studies have confirmed and as many of us clinicians have long suspected, has an even higher prevalence than previously quoted of dementing disorders, is the major cause of this. We are thus challenged by, as Bernard Issacs used to call it, "the survival of the unfittest," as well as the oPtimistic approach of "bringing life to years," as John F. Kennedy said. The fact is that we, as a society, tend to confuse "treatment" and "cure" (and "prevention"). As the proceedings of the conference which this book represents emphasize, there is considerable work going on about the potential prevention of, or at least the reduction of, symptomatology in these illnesses by interventions genetically, chemIcally, and so forth. However, the more we find out, the more complicated it becomes, and the more heterogeneous Alzheimer's and the related disorders appear to be, not only in their manifestations (as clinicians have long recognized) but also in the individual initiating and underlying processes. For these reasons, absolute preventive techniques or the likelihood of an intervention which will reverse the process in a high proportion of patients, do not appear to be just around the corner.

Anhand der außergewöhnlich hohen Zahl von 3354 in neun Jahren untersuchten Patienten (ambulant, stationär und nachuntersucht) zeigt der Autor multifaktorielle Definitionen des pathologischen psychosomatischen Reaktionsmusters und des diagnostisch-therapeutischen Managements in der Psychosomatik. Die Besonderheit des Buches besteht darin, daß es die ambulanten und stationären Versorgungsstrategien kritisch und anhand einer ungewöhnlichen Fülle beweiskräftigen statistischen Materials untersucht. Als Erhebungsstrukturen werden EDV-reife Fragebögen (verwendet: interdisziplinärer Kurzfragebogen für Ambulanzen; Nachuntersuchungsfragebogen), mit progredienter Differenziertheit konzipiert, von denen der ausführlichste (unverwendet) modellhaft zur Diskussion steht. Da auch Daten zum konsultierenden und delegierenden Mediziner erhoben werden, ist die Basis gegeben, die zu einer Metaanalyse des "wer behandelt (oder nicht) welchen PPSRM-Patienten zu welchem Zeitpunkt, mit welchen Mitteln und welchem Erfolg" verwendet werden.
