Cervical Spine I: Strasbourg 1985

Cervical Spine Research Society European Branch "The Cervical Spine Research Society is an organization of individuals interested in clinical and research problems of the cervical spine. Its purpose is for the exchange and development of ideas and philosophy regarding the diagnosis and treatment of cervical spine injury and disease. The com­ position of the membership should reflect the varying specialities and disciplines dealing with the cervical spine; biochemical, engineering, neurology, neurosurgery, radiology, orthopedic surgery and others", wrote J. William Fielding, New York City, one of the Society'S founding members. In 1984 the membership list contained 118 names. The overwhelming majority of members came from the United States, four from Canada and 9 Europeans, amongst them Mario Boni. He was the organizer, in con­ junction with the members of the Cervical Spine Research Society, of a splendid "International Meeting for the updating on pathology and surgery Preface VI of the cervical spine" held in Pavia (Italy) in May 1982. It was from this meeting that the idea of a European branch of the Society first originated, with the purpose of establishing multi-disciplinary meetings in Europe. The first meeting of the European branch was organized by A. Weidner in September 1984 in Osnabriick (Federal Republic of Germany). Mario Boni was elected president. He was not only the initiator but also the drivinglorce who tirelessly promoted the development of the European branch. Mario Boni's death in 1986 at the age of 64 was a great loss to us.

Cervical Spine II: Marseille 1988

The second common meeting of the European Section and the Cervical Spine Research Society took place in Marseille (France) from June 12 to 15, 1988 and was organized by Rene Louis. More than 130 specialists from every part of Europe, from America and Asia participated, representing, among others, the fields of Orthopedics, Neurosurgery, Traumatology, Neurology, Anatomy, Rheumatology and Radiology. This meeting again was convincing proof of the growing interest which exists in Europe in research into injuries and diseases of the cervical spine. The main topics of this meeting were the subluxation of the lower cervical spine (chapter 1) and the infectious diseases of the cervical spine (chapter 2). Chapters 3 and 4 of this volume deal with degenerative lesions and the upper cervical spine. In chapter 5 experimental reports are presented, so that a good synopsis is provided of our present state of knowledge of diseases of the cervical spine. The European Section of the Cervical Spine Research Society was founded by Mario Boni in 1984, who unfortunately died in 1986 and was proclaimed Honory President of the European Section. There is a European meeting every summer and one every three years with our colleagues from the United States of America, Canada and Japan organized in Europe. The first combined meeting was organized by Pierre Kehr in Strasbourg (France) in 1985 and the presented papers were published in Cervical Spine I (Springer, Wien-New York, 1987).

Cytological screening for the identification of intraepithelial neoplasia of the cervix as a precursor lesion for cervical cancer has been well established as an effective means for decreasing the incidence of invasive carcinoma. Despite these screening efforts, carcinoma of the cervix remains one of the more common malignancies in women and it is the leading cause of cancer death in many countries in the western hemisphere. It is estimated that in 1986 there will still be 14,000 new cases of invasive cancer, with 6,800 deaths in the United States alone. Unfortunately, many of these patients present with advanced disease, posing difficult management problems for the clinician responsible for their care. The treatment of early stage invasive carcinoma of the cervix (lesions confined to the cervix and vagina) remains either radical surgery, radical radiation therapy or a combination thereof This approach is extraordinarily effective in the vast majority of patients. However, there remains a subset of patients with early stage disease that are at high risk for recurrence. Dr Kjorstad (Chapter 2) has identified adenocarcinomas and adenosqua­ mous carcinomas as having a particularly poor prognosis. In addition, patients with more than three positive lymph nodes or with involvement of lymph nodes outside of the pelvis have a very poor prognosis. He has iden­ tified the CEA as a potentially predictive marker for these patients with poor prognosis, especially in patients with adenocarcinomas.

On the other hand, there are fears and misconceptions. I have heard many doctors say "I have never been sued for a section I did, but I have been sued for the section I did not do. " The fear of not having performed a section in my opinion is real, although difficult to prove, and until the public can be educated that cesarean section delivery cannot eradicate fetal death and damage, this fear will remain and will be responsible for some unnecessary cesarean sections. Bruce Flamm and I hope this book will correct misconceptions that have been responsible for many unnecessary cesarean sections. I am still frequently asked the same old question: What is an ideal cesarean section rate? I still give an answer similar to the 1983 answer, perhaps somewhat modified.
