As indefatigable as ever, they bring together the latest in clinical-pathophysiological considera tions for the clinician. The topics run the gamut from the lovely liver to HIV infection, from the pathophysiology of bile flow to comments on antigens in colorectal cancer. It is always easy to publish a first volume, for enthusiasm is high and everyone is anxious to win a place. Bringing out a second volume, the carrying forward of a good idea, is so much harder. In many ways this second volume in the series marks the coming of age of Canadian gastroenterology, as 11 of the 21 contributors currently reside in Canada. Still, the contributions from the United Kingdom and the United States make this an Anglo-American festival. Reading the chapters will make you appreciate the care with which the contributors have garnered recent references to give up-to-date practical infor mation for us all. I enjoyed all that I read. This is now a science with various specialized fields of research. The results are published in different and special journals, proceedings and books often not easily available to those responsible for the daily practical management of the patients with head injuries. Epidemiological investigations are necessary to evaluate the severity and frequency of accidents and injuries. Such studies will stress the importance of analysis of the causes and also the importance of prevention. They are useful for evaluation of the effects of injuries despite management. Moreover, the researchers of the different aspects may need some knowledge of other links in the chain of events at and after an impact. This is particularly evident with respect to the problems of accident and injury, their prevention, reduction, management and the presentation of the most important clinical features in each case for international comparisons. Therefore it is appropriate to let the different specialists briefly discuss and present their aspects of the subject. Moreover, it may facilitate and stimulate the clinicians in studying special fields of interest. It provides not only methodological aspects of the dose-finding methods, but also software implementations and practical considerations in applying these complex methods to real cancer clinical trials. Thus, the book aims to furnish researchers in biostatistics and statistical science with a good summary of recent developments of adaptive dose-finding methods as well as providing practitioners in biostatistics and clinical investigators with advanced materials for designing, conducting, monitoring, and analyzing adaptive dose-finding trials. The topics in the book are mainly related to cancer clinical trials, but many of those topics are potentially applicable or can be extended to trials for other diseases. The focus is mainly on model-based dose-finding methods for two kinds of phase I trials. One is clinical trials with combinations of two agents. Development of dose-finding methods for two-agent combination trials requires reasonable models that can adequately capture joint toxicity probabilities for two agents, taking into consideration possible interactions of the two agents on toxicity probability such as synergistic or antagonistic effects. Another is clinical trials for evaluating both efficacy and toxicity outcomes in single- and two-agent combination trials. These methods are often applied to the phase I trials including MTAs because the toxicity and efficacy for a MTA does not monotonically increase with dose, but the efficacy often increases initially with the dose and then plateaus. Successful software implementations for several dose-finding methods are introduced in the book, and their operating characteristics in practice are discussed. Recent advance of the adaptive dose-finding methods in drug developments are also provided.
Modern Drug use: An Enquiry on Historical Principles

MannAureolus Theophrastus Bombastus von Hohenheim (1493-1541), commonly called Paracelsus, was both one of the most original medical thinkers of the sixteenth century and was the man who made opium (as laudanum), arsenic, copper sulphate, iron, lead, mercury, potassium sulphate, and sulphur part of the pharmacopoeia. A man of many parts, but a pioneer chemist, Paracelsus can be regarded as the originator of a body of work which was the precursor of chemical pharmacology and therapeutics. To no small extent he stands, therefore, as a father figure of the modern pharmaceutical industry. Today's physician who wants to look at that industry since the days of Paracelsus and weigh the great gains against the problems soon encounters difficulties. To diminish them, this Enquiry approaches its subject from historical principles. This gives increased perspective to questions asked late in the boo- these questions being prompted by medical practice outside the industry and some twenty years of drug development activity within it. In antiquity medicines often seem to have been used as part of magic and primitive man thought disease to be due to supernatural forces which he could influence. The legacy remains - and in trying to sort out what is rational in our use of drugs today we have to separate our small bits of science from the ancient magic and from modern commercial pressures and conditioning.

B. ) in a course on electrochemistry to students of energy conversion at the University of Pennsyl- nia. It was there that he met a number of people trained in chemistry, physics, biology, metallurgy, and materials science, all of whom wanted to know something about electrochemistry. The concept of writing a book about electrochemistry which could be understood by people with very varied backgrounds was thereby engendered. The lectures were recorded and written up by Dr. Klaus Muller as a 293-page manuscript. At a later stage, A. K. N. R. joined the effort; it was decided to make a fresh start and to write a much more comprehensive text. Of methods for direct energy conversion, the electrochemical one is the most advanced and seems the most likely to become of considerable practical importance. Thus, conversion to electrochemically powered transportation systems appears to be an important step by means of which the difficulties of air pollution and the effects of an increasing concentration in the atmosphere of carbon dioxide may be met. Cor- sion is recognized as having an electrochemical basis. The synthesis of nylon now contains an important electrochemical stage. Some central biological mechanisms have been shown to take place by means of electrochemical reactions. A number of American organizations have recently recommended greatly increased activity in training and research in electrochemistry at universities in the United States.
