Whilst a support role working alongside registered nurses has been a longstanding feature of the NHS, the contemporary HCA role has become increasingly central to the process of health service modernization. The role is now assuming even greater importance as the ramifications of financial constraints, restructuring and other pressures on the NHS play out. The issue is becoming increasingly relevant as the government has commissioned an independent review into the role of healthcare assistants, the Cavendish Review, which uses this book extensively. The HCA role is unregulated and low paid, but by taking-on direct care tasks from registered nurses, the role has become politically sensitive. The HCA remains a cheap and flexible source of labour, but the unregulated role encourages dilemmas and public scrutiny over risk and patient safety. The book explores how public policy reform of the health service feeds through to impact upon the management and structure of the healthcare workforce. More specifically, the book provides a timely evidence base for the extended and growing use of the HCA role. The book draws upon a multi-method research design from four geographically located hospital trusts in England, which during a three year period saw over 270 staff interviewed, focus groups and interviews with over 100 patients, some 275 hours of ward-based observation, and detailed survey responses from over 3,000 members of staff and hospital patients. Approval of GnRH and its analogs for treatment of prostate cancer, endometriosis, of ovulation indicate the range and precocious puberty and for induction of these agents. of usefulness In order to bring together the leaders in the basic and clinical science of GnRH, a conference was organized on "Modes of Action of GnRH and GnRH Analogs" and held in Scottsdale, Arizona, February 26 to March 2, 1991. The presentations, given as chapters in this volume, show both the advances in the body of information in this discipline and the efforts underway to reduce basic science to clinical practice. The audience was a combination of representatives from universities, government, industry, and physicians in practice. The lively discussions and insightful questions indicated the interest in the topics discussed and frequently served to catalyze planned interactions of the meeting participants. The organizers are grateful to the speakers and poster presenters for their contributions and for the timely preparations of the manuscripts included in the present volume. Modified Inferior Turbinoplasty: A new surgical approach

Breathed air is slowed down, filtered, warmed and humidified by the nose; many mechanisms are involved in this act, which prevent bronchial tubes and pulmonary alveolus from receiving an excessive and sudden air load. It is well known, on the other hand, that breathing through the mouth causes a lot of problems: inflammatory diseases of the upper airways, sleep apnea syndrome, but also, in some cases, bronchial, pulmonary and even cardiac disorders, that are more easily affecting patients not able to breathe correctly through the nose. For this reason, restoring an efficient nasal function is a very important - and in some cases crucial - issue. This book describes in details an innovative surgical technique called modified inferior turbinoplasty, which offers an excellent solution to the problems associated with the lower turbinate hypertrophy. A turbinate (or nasal concha) is a long, narrow, curled bone shelf that protrudes into the breathing passage of the nose. Most surgical interventions treat only the soft parts of hypertrophic turbinates using laser therapy, radiofrequency treatment, and electrocoagulation, but these procedures often lead to relapse. With this technique, in contrast, all anatomic parts of the turbinate are treated, including bone tissue: the surgeon reduces the inferior nasal concha and the internal nasal tissue, and the mucosa is then remodeled with sutures. The modified inferior turbinoplasty allows the complete avoidance of swabs in the nose, which is fundamental for patients well-being and grant them a quicker recovery. The book will be very useful for othorinolaryngologists, plastic surgeons, endoscopic and maxillofacial surgeons.

Alkylation and oxidation show a potential to disrupt the cellular redox equilibrium and cause cellular damage leading to inflammation and even chronic disease. Furthermore, DNA damage can drive mutagenesis and the resulting DNA sequence changes can induce carcinogenesis and cancer progression. Modified nucleosides can occur as a result of oxidative DNA damage and RNA turnover, and are used as markers for various diseases. To function properly some RNA needs to be chemically modified post-transcriptionally. Dysregulation of the RNA-modification pattern or of the levels of the enzymes that catalyze these modifications alters RNA functionality and can result in complex phenotypes, likely due to defects in protein translation. While modifications are best characterized in noncoding ribonucleic acids like tRNA and rRNA, coding mRNAs have also been found to contain modified nucleosides. This book is a valuable resource, not only for graduate students but also researchers in the fields of molecular medicine and molecular biology.
