Best Evidence Structural Interventions for HIV Prevention

by Rachel E Golden Charles B. Collins Shayna D Cunningham Emily N Newman Josefina J. Card

Providing detailed information on structural HIV prevention interventions, this book is intended for health care practitioners and researchers to plan, implement, and evaluate such interventions in their own communities. As defined by the CDC, structural interventions focus on the physical, social, cultural, political, economic, legal, and/or policy aspects of the environment. Designed to reach a large number of individuals, structural interventions usually occur across entire communities, cities, or countries. As a result, the resources required to initiate structural interventions can far exceed those required for smaller-scale behavioral programs. However, changes from structural interventions have the potential to last over time, even after the programs have ended, resulting in effective use of public and private prevention resources. Because the reach of structural interventions is typically larger than that of individual- or group-focused interventions (for example, the 100% Condom Use Program, which was implemented countrywide in Thailand), their influence may be equally—if not more—significant. This book is a resource for health practitioners, educators, and researchers who seek HIV/AIDS structural prevention programs that have been shown to be effective in their regions or for their target populations (e.g. injection drug users, commercial sex workers, or the general public). With extensive case studies, the book classifies interventions according to the desired outcomes (specific behavior or policy changes) so that the reader may focus on examples of programs with similar goals and target populations to their own. Addresses the quintessential public health ethical dilemma regarding which types of environmental changes should be mandatory via legislation and which should be voluntary, promoted via programmatic, practice, and policy change.

The Best For My Child: Did The Schools Market Deliver?

by Fiona Millar

Parent choice, diversity of school provision and the idea of a quasi-market in schools have dominated education policy for the last thirty years since the passage of the 1988 Great Education Reform Act. But has the market worked in the way that was intended? Are schools better? Do we have a fairer school system? Do parents really have choice? Author and journalist Fiona Millar looks at why these policies have proved so seductive to a generation of politicians and seeks to uncover whether we really are doing "the best for all our children". If we are not, what future reform could and should look like." This is an area that has preoccupied me since the early 1990s when my own children's school was one of the first to be named and shamed by Ofsted," explains Millar. "We had first hand experience of how the market worked in practice so this is an area I have followed with close interest every since. Thirty years on from the 1988 Act I feel we are ready for a new approach but that is important to understand what worked and what could be better. I hope my book will help to answer those questions."

The Best of Times: Challenges and Triumphs in British Politi, Economi and Foreign Affairs 2013-2015

by Mark Field

Tensions simmer in the aftermath of 2008’s catastrophic financial crash. As the darkest days of the crisis fade behind us, great obstacles to prosperity remain. Mark Field’s acclaimed first book, Between the Crashes, explored the economic collapse. Now, in his new collection of essays, the City of London MP charts our progress as the economy splutters back to life, untangling the rhetoric of deficit reduction from reality, and meaningful reform from political posture. The Best of Times traces the compromises of coalition government and the emergence of a new anti-establishment sentiment, taking in the Scottish independence referendum, UKIP’s rise and fall, and the unexpected genesis of Corbynism. Conflicts in the Middle East open a dark chapter in foreign affairs while, closer to home, a shock Conservative victory in the 2015 general election ensures a hotly debated EU referendum.Yet, for all the surprises delivered by the past three years, today’s challenges are deeply familiar: shifting global power, generational wars and disillusionment with capitalism still threaten our prosperity. Engaging and unflinching, The Best of Times offers erudite analysis and practical solutions to the challenges facing Britain today.

The Best of Times, The Worst of Times: A History of Now

by Michael Burleigh

In the decades since the end of the Second World War, it has been widely assumed that the western model of liberal democracy and free trade is the way the world should be governed. However, events in the early years of the twenty-first century – first, the 2003 war with Iraq and its chaotic aftermath and, second, the financial crash of 2008 – have threatened the general acceptance that continued progress under the benign (or sometimes not so benign) gaze of the western powers is the only way forwards. And as America turns inwards and Europe is beset by austerity politics and populist nationalism, the post-war consensus looks less and less secure. But is this really the worst of times? In a forensic examination of the world we now live in, acclaimed historian Michael Burleigh sets out to answer that question. Who could have imagined that China would champion globalization and lead the battle on climate change? Or that post-Soviet Russia might present a greater threat to the world’s stability than ISIS? And while we may be on the cusp of still more dramatic change, perhaps the risks will – in time – bring not only change but a wholly positive transformation.Incisive, robust and always insightful, The Best of Times, The Worst of Times by Michael Burleigh is both a dazzling tour d’horizon of the world as it is today and a surprisingly optimistic vision of the world as it might become.

The Best People: Trump's Cabinet and the Siege on Washington

by Alexander Nazaryan

An engrossing look at the Trump cabinet: the scandals, the incompetence, the assault on the federal government, the bungled attempts to impose order on an administration lost in a chaos of its own making.Donald Trump promised a return to national greatness, but each day of his presidency seems to bring a new crisis, a deepening sense of national unease. Why, and how, has he failed his supporters? And how has he, on occasion, bested his detractors? The Best People takes complete measure of the Trump administration, to grasp with clarity the president and his intentions, and how those intentions are being carried out-or subverted-by the people he has hired. Alexander Nazaryan argues that the "assault on the administrative state" promised by Steve Bannon in early 2017 never came. What the American people got instead was Wilbur Ross hauling his tennis pro to confirmation hearing preparations; Scott Pruitt running away from rattlesnakes; Reince Priebus enduring insults from junior White House staffers. And yet, bungling as Trump's cabinet members have been, they have managed to either damage or arrest many of the gears that make government run. They have given away public lands to oil companies and allowed corporate lobbyists to make decisions about what is best for the American people, and have done it all while flying on private jets and dining at the finest restaurants, at taxpayers' expense. Meticulously reported and enthrallingly told, The Best People takes readers inside the federal government under Trump's control, a government assailed by the very people charged to lead it, a government awash in confusion and corruption.

Best Practices for Acknowledging and Addressing Racial and Ethnic Health Disparities in Medical Education (IAMSE Manuals)

by Jacqueline M. Powell Rachel M. A. Linger

This manual is an instructional guide to provide healthcare educators with best practices for acknowledging and addressing racial and ethnic health disparities (REHD) in medical education. As a collaborative effort written by both medical students and educators, this manual examines the impact of race, racism, and ethnic biases on medical care and health outcomes. This book enables readers to understand and apply key terms encompassing health disparities, bias, and cultural humility as an approach to demystify stereotypes, social assumptions and long-held misperceptions that influence the misuse of race in medical teachings. By examining the construct of race, differences between race-based and race-conscious medicine are distinguished. As such, medical educators will be guided to consider the effects of socioeconomic differences, environmental factors, and institutional racism between population categories with regard to healthcare compliance and outcomes. Supported by evidence-based recommendations, this manual provides medical educators, curriculum managers, and institutions with strategies and checklists to improve their medical curricula to ensure a well-defined understanding of race and ethnicity in medicine. This book serves as a resource for medical educators and students as they aspire to become more culturally competent, equity-minded, and inclusive healthcare professionals.
